Online Registration for Southeast Texas Homeschool Prom

Student Information:  A separate registration form must be completed for each student (and guest of student, if applicable.)  If you are paying for a guest of your student, but don't know the guest's name just yet, use GUEST OF for first name and your student's name as last name so I will be able to match payments up and know to hold the spot.  Then when you have the guest's information, make sure to have the homeschooled student's name in the "guest of" field. 

    Parent/Guardian information:
    It would be very helpful for online registration and payment to be done at the same time.

Registration may also be done through the mail. 
Make your check /money order payable to Melanie Watanabe, and drop it in the mail.  Registering on the website is all that is required; no need to send in registration forms.  Please make sure the student's name is on your check/money order.

There will be a $35.00 return check fee. Do not mail cash; we are not accepting cash through the mail for payment.   
                                 
                                                    Include your teen's name on all correspondence, including payment, so the information will be         applied to the correct person.


An email confirmation will be sent to you within 3 days after your completed registration (payment and registration/waiver release forms) have been received.
The Fine Print:

This event is for homeschooled students age 14 as of 8/31/18 through age 19.  All tickets are non-refundable.  You will receive an emailed confirmation of payment and registration, and your student's name will be added to the list of guests.  An adult volunteer at the entrance of the venue will check for your student's name on the guest list and will direct him or her to the dance area.  You will not receive an actual ticket.

Dress Code: 
Ladies:  Special occasion, semi-formal, or formal dress.  Take care to make sure your dress is not too revealing - no plunging necklines, super short skirts, or open midriffs.
Gentlemen:  Tuxedo, suit, or dress slacks and button down shirt.  No jeans.

If an attendee arrives at the prom in attire that does not meet the guidelines, then he or she will be asked to return to the prom when appropriate changes to attire have been made.  The decision of appropriateness is left to the discretion of the prom committee and chaperones.  No refund of prom fees will be given.

Conduct:  For the enjoyment of all students, no inappropriate language or behavior will be tolerated.  If your student leaves the prom prior to 11:00 pm, he or she will not be allowed back into the prom.  All attendees must stay in the designated prom area.

Waiver and release:
In consideration of being permitted to participate in the Southeast Texas Homeschool Prom, all participants and volunteers (referred to as Family) agree to the following:
1. Release, waive, discharge, and covenant not to sue either the Southeast Texas Homeschool Prom, the locations of prom activities, their officers, employees, board members, or other families participating in the prom (referred to as Releasees) from all liability for any loss or damage and any claim or damage on account to the person or property resulting in death of a Family member, whether caused by negligence of Releasees or otherwise Family participating in activities of the Southeast Texas Homeschool Prom or any other activities on the premises of the prom.
2. Family agrees to indemnify Releasees from any loss, liability, damage, or cost Releasees may incur due to the presence of Family in or on the premises of Prom.
3. Family assumes full responsibility for and risk of bodily injury, death, or property damage due to negligence of Releasees or otherwise while in or on the premises of Prom locations and/or while working for any purpose participating in the activities of the Southeast Texas Homeschool Prom.
4. Family expressly agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Texas and that if any portion of the agreement is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
5. Family, in consideration of being permitted to participate in the Southeast Texas Homeschool Prom, for Family, and Family’s heirs, executors, administrators, and
assigns, releases and forever discharges all Releasees, and their heirs, administrators,
and executors of and from any and every claim, demand, action or right of action, of
whatsoever kind of nature, either in law or in equity arising from or by reason of any
bodily injury or personal injuries known or unknown, death or property damage resulting
or to result from any accident which may occur as a result of participation in the
Southeast Texas Homeschool Prom or any activities in connection with the Southeast
Texas Homeschool Prom, whether by negligence or not.
6. Family grants to the Southeast Texas Homeschool Prom and its successors and
assigns, the right to use and publish for educational or other purposes, photographic
likenesses or pictures of Family taken at the Prom or at Prom-related events and
activities.
7. Family releases all officials and professional personnel from any claim whatsoever on account of first aid, treatment or service rendered Family during participation in the
Southeast Texas Homeschool Prom.
8. The parties agree that any claim or dispute arising from or related to this agreement
shall be settled by biblically based mediation and, if necessary, legally binding
arbitration. Judgment upon arbitration decision may be entered in any court otherwise
having jurisdiction. The parties understand that these methods shall be the sole remedy
for any controversy or claim arising out of this agreement or any dispute arising out of
any activities or events associated with the Southeast Texas Homeschool Prom and
expressly waive their right to file a lawsuit in any civil court against one another or
against another Homeschool Prom family for such disputed, except to enforce
arbitration decision.
9. This agreement shall be binding on each member of the Family, their personal
representatives, assigns, heirs, and next of kin.
10. This release contains the entire agreement between the parties to this agreement
and the terms of this release are contractual and not a mere recital.
